Program 01
Children’s Palliative Care Service
Free home visits from paediatric palliative nurses for children living with cancer, HIV and other life-limiting conditions across Jakarta.
Why it matters
Children referred by hospitals and community clinics receive ongoing home care, symptom relief and family support for as long as they need it.
[Replace with the detailed program description from the Rachel House proposal — scope, eligibility, geography, referral partners and clinical protocols.]
Free home visits from paediatric palliative nurses for children living with cancer, HIV and other life-limiting conditions across Jakarta.
Who it is for
Children living with life-limiting illness whose families cannot easily reach or afford ongoing specialist care. Referrals come from hospitals, community clinics and health workers who know the family.
What it costs a family
Nothing. Every visit, every dressing and every dose of pain medication provided through this program is free. It is funded entirely by donations.
How it works
From referral to ongoing care.
Four stages, repeated for every child in the program.
-
Step 1
Referral
A hospital, clinic or community health worker refers a child to our service. There is no fee and no waiting list for pain.
-
Step 2
First visit
A nurse visits the family at home, assesses the child’s pain and symptoms, and agrees a care plan with the parents.
-
Step 3
Ongoing care
Regular visits continue, adjusting medication, dressing wounds, supporting nutrition and coaching the family.
-
Step 4
Bereavement
If a child dies, our team stays with the family — through the funeral and the months that follow.
